Perhaps he had a grey arrow and is green stamina bar is empty ? And you haven't or couldn't make a substitution ?

It's no a death it's injury but player continue a while and then fall, we don't see it because the game continue and camera too.so it happens and we don't see

thats what happens when u play a player in every game with no breaks or rests in a ML

actually, it also happened to me with Zico, i was playing a memorial match with a friend. and so he had no fatigue, i had just scored a goal, and kicked off - zico died in that way (he got a red injury)
